At Sanjeevani Cancer Care Hospital we are equipped with 2 Medical Records Departments (MRD), one in each unit.

Our  prime objective is,  the provision of patient Medical Records in a timely manner to patients and doctors on request,  in order to assist clinicians, allied health professionals and other hospital staff in the provision of quality care to patients.

In addition, MRD is responsible for maintaining medical records in a standardized and professional manner in order to protect patient confidentiality while allowing adequate access to providers in order to promote quality patient care.

Among the various records you have the right to obtain:

• Any notes or records that a provider has created themselves
• Any diagnostic results for which a provider has copies including blood tests, X-rays, mammograms, genetic tests, biopsies, etc.
• Any information provided by another doctor that was used to establish a diagnosis and/or direct treatment 

Documents you need to bring with yourself:

• Original ID proof like Aadhaar card, PAN Card, Voter’s ID, Driving licence, Passport
• Photocopy of ID proof
